FORM Basketball Studio is currently operated by Justin Cederholm, Kāpiti Coast, New Zealand. For privacy questions or requests, email justin@formbasketball.co.nz.
What we collect
Depending on the form you use, we may collect:
- Your name, email address, suburb and connection to basketball.
- Your survey answers about demand, pricing, timing and programme preferences.
- Business, phone and partnership information when you request the sponsorship prospectus.
- Basic technical information generated when the website is used, such as request and security logs.
Please do not submit sensitive information or personal information about a child through these forms.
Why we collect it
We use this information to:
- Assess demand for FORM and shape its pricing, hours, services and programmes.
- Send occasional research, validation and launch updates you have agreed to receive.
- Respond to sponsorship and partnership enquiries.
- Prevent abuse, maintain security and understand whether the website is working.
We will not sell your personal information.
Your choice
Providing information is voluntary. If you do not provide required form fields, we may not be able to add you to the founding list, record your research response or respond to a partnership request. You can unsubscribe from updates at any time.
Who can access it
Access is limited to Justin Cederholm and service providers needed to host, secure and operate the website. The website and database currently use managed cloud infrastructure supplied through OpenAI Sites and Cloudflare. Those providers may process or store information outside New Zealand under their applicable safeguards.
If FORM later connects a CRM, email or analytics provider, this statement will be updated before that provider receives personal information.
Storage, security and retention
We use access controls and managed cloud security to protect stored information. No system can guarantee absolute security. We retain validation and enquiry information only while it remains useful for the purposes above, legal requirements or resolving a dispute, and periodically review it for deletion.
Access and correction
You can ask for a copy of personal information FORM holds about you, request a correction, withdraw from updates or ask us to delete information that is no longer required. Email justin@formbasketball.co.nz. We may need to verify your identity before completing a request.
Changes
We may update this statement as FORM’s systems and services develop. The latest version and its effective date will remain available on this page.
